[ayso45-refs] Youth refs: Please carry a copy of your volunteer form

To all region 45 youth referees,

Please take a moment to print or copy your youth volunteer form from 
eAYSO (http://www.eayso.org) and put it with your referee gear.

Similar to the way coaches must carry a copy of the player registration 
form for every player on their team, you need to have a copy of your 
volunteer form with you.  Your volunteer form is also a medical release 
form, that allows others at the match to seek medical care for you if 
you are injured while refereeing. Referee injuries are very unlikely, 
but let's not tempt the soccer gods ...

Carrying the form is absolutely required when you are refereeing a match 
without one of your parents present. The safest approach is to print a 
copy and carry it with you all the time.
