[ayso45-refs] Please include brief comment when declining an assignment

Refs,

When you have to decline an assignment in WebYouthSoccer, please include 
a brief comment:

    Available before noon not after

or

    Called out of town

This will help the schedulers fit you into a time slot that works for 
you, whenever possible.

And please, please, set/update your referee preferences. If you know 
that you won't be available, mark the date unavailable as early as 
possible.  If you have a time restriction on a particular game day, 
include that in the "Special instructions" field.

Update your preferences as frequently as your schedule changes.  That 
will really help your scheduler volunteers.
