[ayso45-refs] Fitness test for National and Advanced Referees

This is the first fitness test of the 2009 season.  There has typically 
been another one offered in December/January.

Referees working on Advanced and National upgrades can do the upgrade 
requirements (training class, law test, fitness test, assessments, 
service units) in any order. If you are planning on a badge upgrade 
soon, consider taking the fitness test date now if this date fits in 
your schedule.
