[ayso45-coaches] AYSO 45 - Final Equipment Collection and Medals Handout

Hello Coaches,

For teams that participated in the Area tournament this past weekend or
otherwise have not turned in your equipment (keeper jerseys, cones, good
ball bags, pinnies and most important *Registration/Medical Release Forms*),
equipment collection will take place at the following location and times:

1201 St. Joseph Ave
Los Altos, CA

Thursday December 10th - 7:30pm to 8:30pm

Thursday December 17th - 7:30pm to 8:30pm

If you have not picked up your medals, this would also be your window to do
so.

If you are coaching a spring select soccer team and are going to hang on to
your equipment, we still need your *Registration/Medical Release Forms*.

Thank you for a great season and we hope to see you come registration time
next April and May,
